Acne Grading: Understanding the Levels of Acne

Acne is often graded based on its severity using a system that includes Grades 1, 2, 3, and 4. Here’s a detailed explanation of each grade: 

Grade 1 Acne: Mild Acne 

Grade 1 acne is considered mild and usually consists of open comedones (blackheads) and closed comedones (whiteheads). These are typically small, isolated lesions that may be occasional rather than continuous. The skin may appear slightly uneven, and there may be minimal inflammation. 

Grade 2 Acne: Moderate Acne 

Grade 2 acne is characterized by a higher number of lesions, which may include papules (small red bumps) and pustules (pimples with pus). These lesions may be more widespread across the face, and the skin may appear visibly inflamed. Grade 2 acne can be more persistent than Grade 1 acne and may require more targeted treatment. 

Grade 3 Acne: Moderately Severe Acne 

Grade 3 acne is more severe and involves a higher number of inflammatory lesions, including larger papules and pustules. Nodules, which are painful, solid lesions beneath the surface of the skin, may also be present. These nodules can contribute to scarring. Grade 4 Acne: Severe Acne 

Grade 4 acne is the most severe form, often involving a large number of deep, painful cystic nodules and pustules. This level of acne can cover a significant portion of the face and may also appear on the back and chest. The lesions may be interconnected, leading to the formation of abscesses. Severe inflammation and scarring are common with Grade 4 acne, and it can have a significant impact on a person’s quality of life.
